Mounjaro and Smoking: Can Tirzepatide Help You Quit?

Can Mounjaro Help You Stop Smoking?

If you take Mounjaro for weight loss, you may have noticed something unexpected: a reduced urge to smoke. Anecdotal reports and early clinical research suggest that tirzepatide, the active ingredient in Mounjaro, could dampen cravings for nicotine alongside its established effects on appetite.

This does not mean Mounjaro is a licensed smoking cessation treatment. It is not. But the biological overlap between food reward pathways and nicotine addiction is real, and researchers are now investigating whether GLP-1 receptor agonists could one day play a formal role in helping people quit.

How GLP-1 Receptor Agonists Interact with Nicotine Cravings

Tirzepatide works on both GLP-1 and GIP receptors. These receptors are found not only in the gut and pancreas but also in brain regions involved in reward processing, including the mesolimbic dopamine system. This is the same circuitry that nicotine hijacks to create dependence.

When GLP-1 receptor agonists activate receptors in these reward centres, they appear to reduce the dopamine surge that makes addictive behaviours feel satisfying. In animal studies, GLP-1 agonists have reduced nicotine self-administration in rats. In human observational data, patients on semaglutide and liraglutide have reported fewer cigarettes smoked per day without any specific effort to cut down.

What the Research Says So Far

A large retrospective study published in 2023 examined electronic health records and found that patients prescribed semaglutide had significantly lower rates of tobacco use disorder diagnoses compared to matched controls. While this was observational, the effect size was notable enough to prompt randomised controlled trials.

Several clinical trials are now underway investigating GLP-1 agonists specifically for smoking cessation. Results are expected from 2025 onwards. For tirzepatide specifically, no dedicated smoking cessation trial has been published yet, but its dual GLP-1/GIP mechanism may produce similar or even stronger effects on reward pathways than single-receptor agonists.

Patient Reports: What CutKilo Patients Have Noticed

At CutKilo, some of our patients who smoke have mentioned a reduced desire to light up after starting Mounjaro. These reports are consistent with the broader pattern seen across GLP-1 agonist users. Common observations include fewer cigarettes per day without conscious effort, less satisfaction from smoking when it does occur, and a general dampening of the habitual urge to reach for a cigarette.

These are individual experiences, not clinical evidence. But they align with what the published research is beginning to show.

Why Quitting Smoking Matters During Weight Loss Treatment

Smoking and obesity are the two leading preventable causes of death in the UK. Addressing both simultaneously would deliver compounding health benefits. Smoking impairs cardiovascular recovery, reduces insulin sensitivity, and increases systemic inflammation. All of these work against the metabolic improvements that Mounjaro is designed to support.

Many people worry that quitting smoking will cause weight gain, which is a legitimate concern. Nicotine suppresses appetite and raises metabolic rate slightly. However, if you are already on Mounjaro, the appetite-suppressing effect of tirzepatide may offset the increased hunger that typically follows smoking cessation. This creates a potentially useful window of opportunity.

Mounjaro Is Not a Licensed Smoking Cessation Aid

It is important to be clear: Mounjaro is licensed for type 2 diabetes and, in some markets, for chronic weight management. It is not approved by the MHRA, FDA, or any other regulatory body as a treatment for nicotine dependence. Prescribing it for that purpose alone would be off-label and is not something CutKilo offers.

If you want to quit smoking, proven options include nicotine replacement therapy, varenicline (Champix), bupropion, behavioural counselling, and the NHS Stop Smoking Service. These should remain your first-line approach. Any benefit from Mounjaro on smoking cravings should be considered a potential bonus, not a replacement for dedicated cessation support.

Can You Smoke While Taking Mounjaro?

There is no direct pharmacological interaction between tirzepatide and nicotine. Smoking while on Mounjaro will not cause a dangerous reaction, and it does not reduce the effectiveness of the medication for weight loss or blood sugar control.

However, smoking does worsen several conditions that Mounjaro is prescribed to improve. It increases cardiovascular risk, promotes insulin resistance, and can aggravate gastrointestinal symptoms like nausea and acid reflux, which are already common Mounjaro side effects. From a clinical perspective, continuing to smoke while on treatment is working against your own progress.

Practical Steps If You Want to Use This Window

If you are on Mounjaro and noticing reduced smoking urges, there are practical steps you can take to build on that momentum. Tell your CutKilo doctor at your next review, as they can help you plan a structured quit attempt. Contact the NHS Stop Smoking helpline on 0300 123 1044 or visit your local pharmacy stop smoking service. Consider combining Mounjaro with nicotine patches or gum during your quit attempt. Track your cigarette intake honestly so you can measure progress. Avoid replacing cigarettes with high-calorie snacks, since Mounjaro should help control that impulse, but stay mindful.

The fact that your cravings are lower does not guarantee a successful quit. Nicotine dependence is complex and involves behavioural habits as much as brain chemistry. But lower cravings do give you a better starting position than most quit attempts.

What About Vaping and Mounjaro?

The same principles apply to vaping. Nicotine delivered via e-cigarettes activates the same reward pathways that GLP-1 agonists appear to modulate. Some Mounjaro users have reported reduced interest in vaping, though the evidence base is even thinner than for traditional cigarettes.

Vaping while on Mounjaro carries no known drug interaction risks. However, nicotine from any source still raises heart rate and blood pressure, which is relevant if you are using Mounjaro as part of a broader cardiovascular risk reduction strategy.

The Bottom Line

Mounjaro is not a smoking cessation drug. But the science behind GLP-1 receptor agonists and addiction is genuine and growing. If you are taking tirzepatide and finding it easier to resist cigarettes, that effect is consistent with what researchers are seeing in both animal and human studies.

Use that advantage. Speak to your doctor, access dedicated quit support, and treat this as an opportunity to address two major health risks at once. At CutKilo, our doctors can help you plan a weight loss programme that accounts for your smoking status and supports you through both changes.

If you have noticed changes in your smoking habits since starting Mounjaro, we would welcome hearing about your experience at your next consultation. If you are considering managing Mounjaro side effects alongside quitting, our team can advise on the best approach.
